Facilities Space Request

We invite you to complete a Space Request Form to reserve space for a meeting or event. Below is a fillable PDF. Download the PDF, fill in your information, and then send it as an email attachment to [email protected]. You may also print off the form, fill it in and drop it in the mail slot at the Parish Center (808 S. East Ave.) You will be notified when your dates are posted or if there is a conflict.

 

If you are requesting the gym for a meeting or event please have the date approved by Dan Kane, the Athletic Director, by emailing him at [email protected]. Indicate on the space request that your gym date has been approved before submitting the date to the parish office.
